Co Pays and Co insurance are due at time of visit. We will do all we can to collect the insurance based on information provided to us at the time of service.

We will bill your visit directly to your insurance provider if you supply an updated proof of eligible insurance. We are happy to file the claim for you, but the balance is your responsibility if your insurance company or plan does not pay after 30 days from billing.  Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us. We will do our best to provide you with co-insurance and cost information prior to your surgery . You may be required to pay your co-insurance or deductible amount prior to your surgery. Please let us know if you need to set up a payment option.

Medicare patients: As a courtesy we will bill your secondary insurance provider directly. Certain procedures will require you to sign an ABN (Advanced Beneficiary Notice) prior to your procedure. Which states if Medicare does not pay for your procedure you understand that you are responsible for covering these services.

As each insurance policy is different it is ultimately the patients responsibility to know if they need a referral or prior authorization to be seen in our office. Please check with your carrier if you are unsure.

Cash patients can receive a discount for payment at time of service.
